NEW JERSEY CAREGIVER RESOURCE DIRECTORY
INTRODUCTION
This directory attempts to provide a fairly comprehensive listing of the agencies in New Jersey which serve the needs of family caregivers. The resources are organized into 7 major sections: Advocacy Groups, Legal, Financial, Transportation, Respite, Hospice, Home Care. (There are two additional sections with recommended websites and books). These 7 resource categories are the services which family caregivers and professionals agree address the major needs of people caring for loved ones with serious, progressive illness, primarily in the home.
Please note that non-profit agency information changes frequently because of changes in the way government services are organized, and changes in funding. Therefore, inevitably, some of the phone numbers, addresses and websites, and even the names of some agencies, will have changed between the time the directory was prepared and published.
Also, note that other than checking the accuracy of contact information, we have not evaluated the quality of services offered by listed agencies. If an agency is omitted, it may mean we overlooked the agency in our research or were unable to verify the contact information and should not be taken as a negative evaluation.

Note:
There are many different types of organizations listed below. In some cases, the listing is for government offices which provide Medicare, Medicaid and other government-funded assistance. Some organizations provide counselors who help patients and families apply for financial help. Other listings are for help with specific services, such as for medications, or for specific medical conditions (e.g., AIDS).
It's important to note that if a patient or family is receiving service from a social worker, a routine aspect of such care is to help identify possible benefits and financial resources. Since the social worker is likely to have much more experience and knowledge in dealing with such issues than the typical patient or family member (or physician), it would be ideal if all patients and families in financial need had consultation with a social worker, on either a limited or ongoing basis. Such social work services can often be obtained through the disease-specific agencies in the Advocacy section, through hospital and community based counseling and mental health services, home care and hospice programs, or through hospital social work departments.

Patients who are eligible for Medicaid should always apply, since Medicaid provides a wider range of benefits at low cost or free. The program is designed for people with very low income and assets. However, family assets may not be counted in considering a patient's financial status. To apply for Medicaid, contact the Social Services office in your county (see list, at the end of the directory).
RESPITE CARE
Note:
The purpose of respite is to relieve unpaid caregivers of the stress arising from providing daily care. A second goal of respite to provide the support necessary to help families avoid nursing home placement of their loved ones. Such care is always provided on a temporary basis; typically no more than several weeks of full-time respite per year, or much longer periods of part-time relief per day. Services may be provided on an emergency basis, as well as in a more routine, scheduled way. Respite services may include providing or arranging for a homemaker/home health aide, temporary adult day care, temporary care in licensed medical facilities such as nursing home or hospitals, and private duty nursing.
See below for County listing of providers of State Respite Care Program services and funds. The advantage of always first calling the designated state-funded agency in each county is that there are funds, up to $3,000 per year per family, which can be accessed for respite care. Families may be asked to share the cost of services, depending on income.

TRANSPORTATION
Note:
Caregivers and patients often need help with transportation, primarily for doctor’s visits, but also for shopping and errands. For each county, there is a designated transportation service for the ill, elderly and disabled, which is supported in part or whole by state funds. (In the listing below, the state-designated agency in each county is marked *.) However, please also note that from one county to another, the availability and conditions of such services vary greatly.
For people with Medicaid, for whom transportation for medical purposes is usually covered, there are Medicaid-specific agencies. Such services can be accessed by calling the local Board of Social Services office (see list at end of Directory), or simply calling any of the state-funded transportation services and being referred to that county’s Medicaid-specific service. For those over 60, the NJ Ease number (877 222-3737) is also a good starting point.
For specific diseases, e.g., Alzheimer's or cancer, the disease-specific support agency in each county (e.g., for cancer, Cancercare) will often provide transportation services.
There are also smaller transportation services run by each town and by small non-profit agencies. The town hall (town clerk) in each specific community will usually have information about town-specific transportation services.
In some communities the American Red Cross chapter is the main state-funded provider of transportation services or has available volunteer drivers.
In all cases, the transportation provided has limitations. Agencies usually require advance notice of a week or more, require that patients be fully able to get in and out of the vehicle or have help to do so, require that patients be waiting curbside for pick-up, and only provide transportation on limited days or hours.

Note:
Hospice may be viewed as a form of home care, although it is also sometimes provided outside of the home. Hospice is a philosophy of care which enhances the quality of the last phase of life by providing aggressive pain management and symptom control, and emotional and spiritual support to the patient and family.
Medicare-reimbursed hospice services include physician home visits, home health aide and homemaker services (e.g., dressing, bathing, personal needs), chaplain visits for patient and family, social work and other counseling, bereavement counseling, medical equipment (e.g., hospital beds used at home, bandages and catheters), medications for symptom control and pain relief, volunteer support, physical, speech and occupational therapy, dietary counseling, and short stays in nursing homes and inpatient units, as well as services provided in a residence the hospice runs.
